We report a case of hypoplastic left heart syndrome and with subsequent aortopathy and then found to have hereditary haemorrhagic telangiectasia/juvenile polyposis syndrome due to a germline SMAD4 pathologic variant. The patient’s staged palliation was complicated by the development of neoaortic aneurysms, arteriovenous malformations, and gastrointestinal bleeding thought to be secondary to Fontan circulation, but workup revealed a SMAD4 variant consistent with hereditary haemorrhagic telangiectasia/juvenile polyposis syndrome. This case underscores the importance of genetic modifiers in CHD, especially those with Fontan physiology.

Desiccation following prolonged air exposure challenges survival of aquatic plants during droughts, water drawdowns, and overland dispersal. To improve predictions of plant response to air exposure, we observed the viability of vegetative fragments of 10 aquatic plant species (Cabomba caroliniana, Ceratophyllum demersum, Elodea canadensis, Egeria densa, Myriophyllum aquaticum, Myriophyllum heterophyllum, Myriophyllum spicatum, Potamogeton crispus, Potamogeton richardsonii, and Hydrilla verticillata) following desiccation. We recorded mass loss, desiccation rate, and plant fragment survival across a range of air exposures. Mass loss accurately predicted viability of aquatic plant fragments upon reintroduction to water. However, similar periods of air exposure differentially affected viability between species. Understanding viability following desiccation can contribute to predicting dispersal, improving eradication protocols, and disposing of aquatic plants following removal from invaded lakes or contaminated equipment.

Approximately half of the variation in wellbeing measures overlaps with variation in personality traits. Studies of non-human primate pedigrees and human twins suggest that this is due to common genetic influences. We tested whether personality polygenic scores for the NEO Five-Factor Inventory (NEO-FFI) domains and for item response theory (IRT) derived extraversion and neuroticism scores predict variance in wellbeing measures. Polygenic scores were based on published genome-wide association (GWA) results in over 17,000 individuals for the NEO-FFI and in over 63,000 for the IRT extraversion and neuroticism traits. The NEO-FFI polygenic scores were used to predict life satisfaction in 7 cohorts, positive affect in 12 cohorts, and general wellbeing in 1 cohort (maximal N = 46,508). Meta-analysis of these results showed no significant association between NEO-FFI personality polygenic scores and the wellbeing measures. IRT extraversion and neuroticism polygenic scores were used to predict life satisfaction and positive affect in almost 37,000 individuals from UK Biobank. Significant positive associations (effect sizes <0.05%) were observed between the extraversion polygenic score and wellbeing measures, and a negative association was observed between the polygenic neuroticism score and life satisfaction. Furthermore, using GWA data, genetic correlations of -0.49 and -0.55 were estimated between neuroticism with life satisfaction and positive affect, respectively. The moderate genetic correlation between neuroticism and wellbeing is in line with twin research showing that genetic influences on wellbeing are also shared with other independent personality domains.

Background
Drugs of dependence cause dopamine release in the rat striatum. Human neuroimaging studies have shown an increase in dopamine in the equivalent region in response to stimulants and other drugs
AimsWe tested whether opioids provoke dopamine release and its relationship to the subjective experience
MethodIn two combined studies 14 heroin addicts on methadone maintenance treatment underwent two positron emission tomography brain scans of the dopamine system using [11C]-raclopride following an injection of placebo and either 50 mg intravenous diamorphine or 10 mg subcutaneous hydromorphone in a double-blind, random order design
ResultsBoth opioids produced marked subjective and physiological effects, but no measurable change in [11C]-raclopride binding
ConclusionsThe absence of a dopamine response to opioid agonists contrasts with that found with stimulant drugs and suggests dopamine may not play the same role in addiction to opioids. This questions the role of dopamine in the subjective experience of heroin in opioid addicts

Hormesis defines an effect where exposure to a low dose of a toxic agent results in a beneficial response. It has been described in organisms exposed to low-dose radiation, heat stress, and chemicals. The effect is characterised by a J-shaped dose–response as opposed to a linear dose–response. Confirmation of the general phenomena of hormesis has proved difficult due to the lack of appropriate methodology and the absence of well-defined mechanisms to support the experimental observations. In the nutritional field there are few reports of its existence. The clearest illustration of the effect is seen in animals that are energy restricted when there is a clear benefit in the reduction of age-related disease, and an extension of maximum lifespan. DNA microarray experiments have shown that there is a down regulation of the stress-response genes that are up regulated through the ageing process. Electrophilic phytochemicals, that have been shown to have beneficial health effects at low doses, up regulate the antioxidant–electrophile response element. This probably occurs through an alteration in the redox state of the target cells which causes activation of protein kinases, the activation of the Nrf2 transcription factor and the up regulation of the phase II enzymes, similar to responses that occur under mild chemical stress. This situation might enable organisms to adapt to stress such that the effects of a subsequent exposure to a harmful challenge are reduced. There may be a permanent alteration in cellular homeostasis, or redox state, if the low level exposure is maintained. It remains to be proven if such a situation occurs in response to chronic low-dose exposure to dietary phytochemicals such that the target cells are better able to respond to a subsequent stress challenge.
Brain opioid receptor binding in early abstinence from opioid dependence: Positron emission tomography study

Background
Although opioid receptor function in humans is clearly reduced during opioid dependence, what happens to the receptor in early abstinence is not understood.
AimsThis study sought to examine changes in opioid receptor availability in early abstinence from opioid dependence.
MethodTen people with opioid dependence who had completed inpatient detoxification and 20 healthy controls underwent [11C]-diprenorphine positron emission tomography. Clinical variables were assessed with structured questionnaires. Opioid receptor binding was characterised as the volume of distribution of [11C]-diprenorphine using a template of predefined brain volumes and an exploratory voxel-by-voxel analysis.
ResultsCompared with controls, participants with opioid dependence had increased [11C]-diprenorphine binding in the whole brain and in 15 of the 21 a priori regions studied.
ConclusionsThis study suggests that opioid receptor binding is increased throughout the brain in early abstinence from dependent opioid use. These data complement the findings in cocaine and alcohol dependence.
Profile Changes and Self-sputtering during Low Energy Ion Implantation.

Continued device scaling requires the formation of ever-shallower junctions with low resistance. A desirable option to form these junctions is still the use of conventional ion implantation. However in order to meet the junction depth/sheet resistance goals, a strong reduction in implant energy and increase in implant dose is required. Earlier work for B and BF2-implants, has suggested that during low energy ion implantation self-sputtering may become an important factor influencing/limiting the retained dose. The basic mechanism for the selfsputtering with increasing dose is the increasing dopant concentration at the surface leading to an increased probability for re-emission by the sputtering process. Simple models describing ion retention in combination with sputtering are based on this concept and indeed predict a selfsputtering process limiting the final retained dose. Unfortunately the theoretical calculations only predict a significant sputtering at doses >51016 at/cm2 whereas experimental results already show a limit in retained dose at 5 1015at/cm2.
In order to confirm the experimental data, low energy B, BF2, As and Sb implants have been made. Dose retention was monitored using nuclear reaction analysis and RBS whereby details of the dopant profile (redistribution) were studied using high resolution SIMS. For As and Sb no self-sputtering up to a dose of 1 1016at/cm2 can be found. For B a small dose loss (<10%) is seen significantly below the literature data. For BF2 a 20 % dose loss is observed. None of the SIMS profiles provide sufficient evidence for enhanced B-surface migration as required to explain the enhanced self-sputtering. On the other hand such a surface migration is reminiscent of the observations in SIMS whereby also an enhanced mobility of B during ion irradiation is required to explain the anomalous B-surface peak in many SIMS profiles. Based on the SIMS profiles a component sputter yield for B can be derived which is significantly higher than the matrix sputter yield suggesting a weak bonding of the segregated species leading to a reduced surface binding energy and thus enhanced sputtering yield.

There is intense antimicrobial use in long-term-care facilities (LTCFs), and studies repeatedly document that much of this use is inappropriate. The current crisis in antimicrobial resistance, which encompasses the LTCF, heightens concerns of antimicrobial use. Attempts to improve antimicrobial use in the LTCF are complicated by characteristics of the patient population, limited availability of diagnostic tests, and the virtual absence of relevant clinical trials. This position paper recommends approaches to management of common infections in LTCF patients and proposes minimal standards for an anti-microbial review program. In developing these recommendations, the position paper acknowledges the unique aspects of provision of care in the LTCF.
